Care homes - need to be funded in remote and rural areas to prevent bed blocking in hospital. Currently many care homes are closing across the Highlands as they are no longer financially viable - this needs to be looked at urgently or patients/folk will end up being cared for at the end of life far away from their lifelong communities with complete loss of social contacts and wellbeing.
As an unscheduled care analyst in NHS Highland, I see and work with data that evidences the ever increasing pressure on 'flow' and the challenges to discharging patients in a timely manner.
